Your child may qualify for Health Check (Medicaid) or NC Health Choice – free or low-cost health insurance for children and teens under 19 years old (up to 21 for Medicaid). Even children in a family of four with working parents earning $47,100 a year or slightly more may qualify. The programs also cover children living with grandparents, other family or friends. Your children need a valid social security number to qualify for these programs. Families with children who are U.S. citizens or lawfully residing in the U.S. are encouraged to apply.

It’s easy to apply!

To apply in person, go to the local department of social services (DSS). To apply by mail, go to www.ncdhhs.gov/dma/medicaid/application.htm and download an application. Mail the completed form and copies of requested documents to your local DSS.

Stay enrolled!

Once your child is covered by this insurance, it will lasts for 12 months. Changes to your household income will not change the insurance. At the end of 12 months, DSS will review your household situation to see if you still qualify. The caseworker will let you know if any information is needed.
