Make each doctor's visit work for you!

Before the visit

Write down your questions and concerns. If it's a long list, ask for a longer doctor's visit. If your child has a special condition, learn about any care your child may need tp stay healthy. If your child sees specialists, tell them to send reports to your child's Medical Home.

On the day

Pack some books, toys, snacks, and your list of concerns. Bring your child's regular medicines, insurance card, and immunization (shots) record. If you want help, ask a friend or family member to join you.

At your medical home

Relax! Ask questions, especially if something is not clear. Take notes. Remember, both you and the doctor want the very best for your child! Partner with your doctor to plan your child's care. Talk about services that may benefit your child. Decide when to schedule your child's next visit.
