Tips for ordering materials

• Fill out the form completely. Use your street address for delivery.
• Materials listed on the Order Form are packaged in set quantities. Note that some items are are available only in limited quantities. Posters may be ordered in any quantity needed.
• Allow 3 weeks for delivery.
• Order enough materials to last at least 2 months.
• If you are going to a health fair, order no more than half of the expected attendance.
• Keep a copy of your order.
